Types of Final Expense Plans For
You
You may qualify for one of three final expense plans:
Level Final Expense Plan: Provides an immediate full Death Benefit and an Accelerated Benefit.
You can receive up to 50% of the Death Benefit if you are diagnosed as having a terminal medical
condition with a life expectancy of 12 months or less.
Graded Final Expense Plan: For nonaccidental death, the benefit during the first policy year is 30%
of the Death Benefit, 70% in the second year and 100% thereafter.
The full Death Benefit will be paid for accidental death in all years.
Modified Final Expense Plan: For non-accidental death, the benefit during the first 2 policy years
will be a premium refund plus 10% interest.
The full Death Benefit will be paid for accidental death in all years.
Optional Accidental Death Benefit: Choosing this low cost rider will DOUBLE your Death Benefit if
death occurs as a result of an accident.
Your Death Benefit is TRIPLED if the accident occurs from riding as a fare-paying passenger on a
common carrier such as a commercial airliner, train, bus, boat or ship, subway or streetcar.
This rider is only available on the Level and Graded Plans.

Your Final Expense Effective
Date
Insurance will become effective once the following conditions have been met: Receipt of application
and full modal premium payment, approval by The Insurance Company, and the Proposed Insured is in the same
condition of health as described in the application when the final expense policy is delivered to you.
All Final Expense Companies retain the right to reject any application which does not meet its
underwriting requirements.
